Over the past 10 to 15 years, medicine in the United States has become more specialized, which means more medical residents are choosing areas of medicine such as radiology, anesthesiology, and cardiology instead of traditional fields like internal medicine, family practice, and pediatrics. It's a trend that many experts believe may have dire results for our medical system and patient care (see "The Primary-Care Conundrum," page 95). But medical students, who leave med school with an average of $140,000 of debt, say the incentives to specialize are too great to ignore. Dr. Kelly Fitzgerald, a fourth-year resident at the University of Colorado, always assumed she would specialize, even after a three-year pediatric residency. "There is more and more pressure in primary care to increase the volume of patients seen within a clinic day while still providing the same caliber of care," says Fitzgerald, who ultimately elected to go into anesthesia. "Plus, the hours for general pediatricians are demanding. The doctors need to be available around the clock, yet they are not financially compensated for this the majority of the time."
Beyond the salaries, which can be triple that of a primary-care doctor, specialists often work fewer hours, endure fewer nights on call, and spend less time building a private practice. In short, specializing offers better quality of life. "Since the mid-1990s, the number of University of Colorado School of Medicine graduates specializing in primary care has gone from more than half to around 40 percent today," says Richard Krugman, longtime dean of the school.
The Listings
Each listing includes the physician's name, the number of times he or she has been on the Top Docs list in that category in the past (if applicable), the hospitals at which the physician has privileges, the doctor's office address, and the best phone number for a patient to call for an appointment. We also note if a doctor is not currently accepting new patients.
